1. Did you live within 10 miles of the  Rocky Flats Plant between 1952-1992?

2. Whether or not you have had health problems, if you lived in close proximity to Rocky Flats we want to hear from you. Please add your name to our list.

3. A health survey completed by residents, can establish the need for medical monitoring. NO HEALTH STUDIES HAVE BEEN DONE ON ROCKY FLATS RESIDENTS.
